Field guide / the medicine

# What is tirzepatide, exactly?

The long name is just the ingredient’s name. The product name and the reason for treatment matter too.

Keep these questions close

Name the exact product.

Ask what it is meant to treat.

Ask who makes it.

Pause. Ask. Check the answer.

## Keep the ingredient and the brand apart.

The Food and Drug Administration (FDA) reviews medicine products for specific uses.

### The ingredient is tirzepatide.

Tirzepatide works like signals from the gut that help control appetite and blood sugar.

These signals are called hormones. They carry messages in the body; you do not need their names to ask about care.

### The brand tells you the approved use.

As of September 26, 2026, tirzepatide (Mounjaro) has approval for blood sugar control in type 2 diabetes, used alongside diet and exercise.

Diabetes affects how the body handles blood sugar.

Mounjaro also has approval to lower the chance of heart attack, stroke or death from heart or blood-vessel disease in adults with type 2 diabetes at high risk.

Weight loss is not an approved use of Mounjaro as of that date.

Tirzepatide (Zepbound) has approval for long-term weight treatment in adults with obesity, or overweight plus a related health problem.

Obesity is a medical condition linked to excess body weight.

For adults who have obesity, Zepbound is also approved to treat obstructive sleep apnea that is moderate to severe.

Sleep apnea means breathing keeps stopping during sleep. Obstructive means the passage for air is blocked.

The Zepbound label pairs these uses with fewer calories (food energy) and more physical activity.

Ask which condition the care plan would address.

## A pharmacy-made version is a different question.

For compounding, a pharmacist or doctor makes medicine suited to the needs of an individual patient.

Compounded medicines had no FDA approval as of September 26, 2026.

Before sale, the FDA does not check each compounded product for quality, safety, or how well it works.

The rules allow compounding for certain patient needs under specific conditions.

## The shortage no longer allows routine copies.

As of September 26, 2026, tirzepatide was absent from the FDA’s national shortage list.

The FDA confirmed the shortage was over on December 19, 2024.

After the shortage ended, the FDA gave pharmacies that made tirzepatide copies time to stop relying on the shortage rules.

That extra time for ordinary compounding pharmacies was to end February 18, 2025, but the FDA extended it until the court ruling on March 5, 2025. Larger facilities making batches had until March 19, 2025.

The law restricts routine copies of a medicine already on the market.

A medicine made to order may avoid the copy restriction when its prescriber judges that a change would make a meaningful difference to a specific, named patient.

A lower price alone does not qualify as that kind of change.

As of September 26, 2026, there was no FDA-approved generic tirzepatide.

Ask for the reason

If compounding is proposed, what particular need would it meet for me?
