Field notes / what to avoid

# Read the limits before the pitch.

A good conversation leaves room for a “no”. Bring up your health history, and expect an answer that goes beyond the advert.

## Bring up the health history that matters.

### Name thyroid cancer in the family.

The thyroid is a gland in the neck. A gland is a body part that makes substances the body uses.

In the labels checked on September 26, 2026, tirzepatide has a boxed warning about thyroid tumours found in rats.

The FDA (US Food and Drug Administration) uses a boxed warning as its strongest warning format.

Whether tirzepatide causes these tumours in humans is unknown.

The labels rule tirzepatide out if you or a family member has had the rare thyroid cancer called medullary thyroid cancer.

Medullary names the particular cancer type. If you know of thyroid cancer in your family but cannot name its type, ask your prescriber to review the records.

The labels also rule tirzepatide out for people with MEN 2, the name of a rare illness that runs in families.

Ask whether a doctor has named this illness in your family. If you are unsure, bring any family medical records and ask the person reviewing your health to check them.

They also rule it out after a serious allergic reaction to tirzepatide or an ingredient in the product.

### Talk about stomach problems and past belly illnesses.

The pancreas and gallbladder are organs inside your belly that help with digestion, the breaking down of food.

Common problems in tirzepatide (Zepbound) trials included nausea, diarrhea, vomiting, and constipation.

Nausea means feeling sick to your stomach.

The label says severe gastroparesis, a stomach that empties very slowly, is a reason Zepbound is not recommended.

Reports after use describe kidney damage in people who lost too much fluid because of stomach problems such as vomiting or diarrhea.

Ask what to do if vomiting or diarrhea continues, and how to get help if you cannot keep fluids down.

The label also reports gallbladder problems, including swelling and gallbladder removal, and serious swelling of the pancreas.

Mention any past gallbladder surgery or a diagnosis of inflammation, meaning swelling, in either organ. Bring your medical records if the names are unfamiliar.

### Bring a medicine list and pregnancy plans.

The Zepbound label says tirzepatide can reduce how well birth control pills work after starting or increasing treatment.

Ask your prescriber to walk through the label’s backup birth control advice as it applies to you.

The label calls for stopping Zepbound when pregnancy is recognised.

The labels warn that blood sugar can fall too low when tirzepatide is taken together with insulin or certain other diabetes medicines.

They also warn that eye damage caused by diabetes can worsen in people who already have it.

Ask which changes in blood sugar or eyesight need a call or urgent help, and who to contact.

They also warn that stomach contents can enter the lungs during general anesthesia (being put fully asleep) or deep sedation.

Sedation means medicines used to make you less awake for a procedure. Tell the surgical team about the medicine and ask for their plan.

## Do not wave away a severe reaction.

A severe allergic reaction may make breathing or swallowing hard, swell the face or tongue, or cause fainting.

For severe allergic symptoms, MedlinePlus directs people to call their local emergency number or go to an emergency room.

The Zepbound label lists lasting or severe belly pain, sometimes reaching the back, as a sign of swelling in the pancreas.

Ask the person providing care when to seek prompt help and how to reach care between visits.

## Pause when the offer makes these claims.

### No prescription required.

As of September 26, 2026, federal law required a valid prescription before a prescription medicine could be dispensed.

[Prescription requirement · US Code](https://uscode.house.gov/view.xhtml?req=granuleid:USC-prelim-title21-section353&num=0&edition=prelim)

### Approval claimed for a compounded product.

As of September 26, 2026, a claim that a compounded drug had FDA approval was false.

A place being “FDA registered” means it is entered in the agency’s records; that does not give its medicines FDA approval.

### A research label used for treatment.

The FDA warns against treatment with tirzepatide bearing a “for research purposes” label or a “not for human consumption” label.

These products have unknown quality and may be harmful.

As of September 26, 2026, no approved generic tirzepatide existed.

### A pharmacy name you cannot verify.

The FDA has found fraudulent labels for compounded tirzepatide that name nonexistent pharmacies or pharmacies that did not make that medicine.

Check the pharmacy name with the state board. Ask for an explanation of any mismatch.
